For the information recorded to be of value it must be an accurate reflection of what has taken place, there should not be ambiguity and there must be clear separation between opinion and fact.
It is also important to consider the legal implications of information being recorded incorrectly or in a misleading way. Further to this the implications around the storage of information relating to clients must be considered.
